What is Continuity of Care (CoC)? The American Academy of Family Physicians notes that CoC is “Concerned with quality of care over time. It is the process by which the patient and his/her physician-led care team are cooperatively involved in ongoing health care management toward the shared goal of high quality, cost effective medical care.”

The resource that guides the G-SPAN breastfeeding priorities is The Continuity of Care in Breastfeeding Support Blueprint from the National Association of County and City Health Officials. The Blueprint, developed with a public health lens, aims to increase local capacity to implement community-driven approaches to support breastfeeding, centered on the needs of populations disproportionately impacted by structural barriers that lead to low rates of breastfeeding.

Healthcare systems provide a unique opportunity for impacting initiation and duration of breastfeeding among Georgia families. They also employ a high quantity of female employees who can be supported in breastfeeding in the healthcare setting.
